I did a bit of swatching with my DMC thread to see how it would knit up and figure out my gauge. I started out with US size 000 needles, but the second try on size 0s looked much better. My gauge turned out to be 9.5 stitches and 14 rows per inch. I am making this just as I would a full-size version -- as a stockinette stitch tube which I will close up on both ends and fringe. I measured a regular bookmark that appeared to be an appropriate size -- 2 by 8 inches. Then I calculated the stitches to cast on and where to place the stripes. Here's the plan for now:
Provisonally cast on 32 sts, divide 8 sts per needle.
Knit 14 rws MC, (1 rw CC, 2 rws MC, 1 rw CC, 14 rws MC) x 5.
Tweak in progress as needed. ;)
